Insurance Information
Dr. Carla Bauman and her staff have the expertise to offer you the most proven treatments for your skin and a satisfying experience that reflects our commitment to exceptional dermatologic care. We accept many (but not all) provider network plans from these carriers. We are not in network with Medicaid, Molina, Apple Health, and Ambetter. Please contact our office to verify acceptance of your plan. Also, please note, many HMO insurances require a referral from you primary care provider (PCP) and/or insurance company before you can be seen. Please confirm coverage and whether you need a referral with your health insurance company.

Privacy Policy
Bauman Dermatology respects your privacy. We understand that your personal health information is very sensitive. The law protects the privacy of the health information we create and obtain in providing care and services to you. Your protected health information includes your symptoms, test results, diagnoses, treatment, health information from other providers, and billing and payment information relating to these services. We will not use or disclose your health information to others without your authorization, except as described in this Notice, or as required by law.
Medical Appointments
Our office asks for a 48 hour notice to reschedule or cancel appointments. Patients who do not show up to a scheduled appointment, without a cancellation phone call 48 hours prior to their appointment, will be subjected to a $95 No Show Fee.
Surgery Appointments
Our office asks for a 48 hour notice of cancellation of cosmetic appointments. Patients who do not show up to a scheduled appointment, without a cancellation phone call 48 hours prior to their appointment, will be subjected to a $150 No Show Fee.
